WISEWOMAN (Well-Integrated Screening and Evaluation for Women Across the Nation)

WISEWOMAN provides free health screenings and coaching to help you make small, healthy changes in your life.

WISEWOMAN helps low-income, uninsured, and underinsured women ages 35-64 make healthy changes by setting small goals.

The Program includes:

  • A brief health intake screening (glucose, cholesterol, blood pressure, etc.)
  • Goal-setting and planning – you decide which behavior you want to focus on to improve your health (anything from eating more fruits and vegetables, to going on a 15 minute walk three days per week, to being more diligent about taking your blood pressure medication, and more)
  • Consistent follow-up with a health coach, including text or phone support
  • Health coaches may refer you to community-based lifestyle programs, such as the Diabetes Prevention Program, and WISEWOMAN will pay for it
  • A follow-up evaluation to measure progress.

In 2022, with support from United Way of Southeast Michigan, WISEWOMAN became more than just a program for those involved. WISEWOMAN health coaches have become a safe, reliable, and empowering influence for participants to focus on what’s most important to them. “My calls with my health coach have been the catalyst for change for me. My whole mindset has shifted. My success is spilling over into other parts of my life too. I’m much happier with where I am than where was a few months ago. I am so grateful,” says Benita T., a WISEWOMAN participant.

Bernice kicked off 2023 – the year of healthy – by enrolling in the WISEWOMAN program. Now 7 months later, Bernice has reached all her health goals and is already sharing what she learned with family and friends in order to help make the community around her healthier. “My goal was to be healthy, wealthy, and wise!” says Bernice. “This program helped me actually get started.” she continues.

The NKFM currently partners with Kent County Health Department to offer WISEWOMAN.
