Do you have kidney disease and want help managing it?

Kidney Personal Action Toward Health (Kidney-PATH) is an 8-week workshop for participants who are in various stages of Chronic Kidney Disease. The first six weeks focus on overall healthy living, with two additional sessions that cover kidney specific content.

Each week, you will set simple, realistic goals to help improve your health. Before Week 1, an information session will be held to provide an overview, walk through registration forms, cover Zoom basics needed to participate, and share helpful community resources.

The program is offered in person and online. Online requires device with internet access and camera.

The first six weeks were developed by Stanford University under the program name: Chronic Disease Self-Management Program. The last two weeks were developed by the NKFM.

What Participants are Saying:

“The two leaders were very understanding and patient. I never felt rushed. They both understood our many concerns even when technical issues came up. I now feel much more confident and somewhat at ease with my condition and I hope all the other attendees feel the same.”

“We learned a lot from each other. We got tips and suggestions from other members of the group that we plan to use ourselves. Knowing that we are not alone in this journey is comforting. Our leaders taught us a lot about taking care of ourselves resulting in giving us a better outlook on our future.”
