2025 marked the 70th anniversary of the National Kidney Foundation of Michigan — seven decades of improving kidney health, expanding prevention efforts, and supporting communities across the state.

70 Years of Impact: Kidney Health Initiatives (KHI)

For decades, our KHI team has transformed kidney health in Michigan through statewide screening efforts, community partnerships, and prevention strategies. Their work continues to advance early detection and support for individuals at risk of chronic kidney disease.

HOW THEY BEGAN

  • The Patient Services department has existed since the NKFM began.
    • Goal: Support individuals and caregivers affected by CKD through innovative education programs and services
  • Health Systems Interventions began in 2004.
    • Goal: Work with health systems, clinics, and health plans to increase screening for CKD.

MILESTONES

  • In 2024, the KHI division was established to bring Patient Services and Health Systems Interventions together.
    • Increased partnerships across Michigan to boost CKD screening. 
    • More screening events with new partners, serving diverse populations.
    • Launched The Big Ask, The Big Give workshops to educate about living donation. Formed a Support Group for people living with kidney disease.
    • CKD-focused programs: Kidney PATH (since 2014), CKD-specific nutrition info, and Cooking Matters at the Store
    • 18 youth attended Kids Camp in 2025, including 8 new campers.

For seven decades, the NKFM has been empowering Michigan communities to take charge of their health. The Health Empowerment Programs are an example of how we meet people where they are with flexible, personalized support that makes healthy living possible for everyone.

The Health Empowerment Programs, or HEP, part of NKFM’s Disease Management & Prevention Programs. HEP is designed to offer flexible, personalized health programs.

  • NKFM’s Data Analytics Team started as a small administrative group, handling data entry, donations, and memorials.

FROM DATA ENTRY TO DATA STRATEGY

  • Today, the team manages databases, online forms, grant reporting, and analytic dashboards. They turn information into insights that guide NKFM programs and funding decisions.

70 Years of Impact: Disease Management Programs - Diabetes Prevention

The Diabetes Prevention Program (DPP) is one of NKFM’s flagship prevention efforts, helping Michiganders reduce their risk for type 2 diabetes and kidney disease.

WHERE IT ALL BEGAN

  • NKFM launched the Diabetes Prevention Program in 2012 and became a CDC-Recognized Provider by 2014.
  • The goal: to prevent type 2 diabetes, a leading cause of kidney disease, through education, community, and lasting lifestyle change.

LEADING BY EXAMPLE

  • Our first-ever DPP class included NKFM staff, coached by our now CEO — a true example of “practice what you preach.”
  • From the start, we’ve been committed to making prevention personal, practical, and accessible to all.

A DECADE OF IMPACT

  • Over the past 10+ years, NKFM’s DPP has empowered more than 5,000 participants statewide!
  • We’ve built strong partnerships with provider practices, health systems, and health plans to expand our reach and help more Michiganders prevent diabetes before it starts.

BREAKING BARRIERS

  • In 2017, we launched DPP en Español, becoming national leaders in offering culturally tailored programs that meet the unique needs of diverse communities.

COLLABORATION IN ACTION

  • In 2025 alone, 16 referral partners helped connect over 3,770 people to our DPP. This is proof that collaboration and community truly drive impact!

THE FUTURE OF PREVENTION

  • We’re continuing to grow our referral network so even more Michiganders can access the DPP and discover how small, sustainable lifestyle changes can prevent chronic disease.

70 Years of Impact: Community Integrated Services

Community Integrated Services (CIS) is NKFM’s hub for connection, helping individuals access the programs, resources, and support they need to thrive.

WHERE IT ALL BEGAN

  • The Community Integrated Services (CIS) department was officially established in 2024, after years of building referral systems and connecting people to NKFM programs (resource navigation).
  • Its mission: Connecting people to what they need.

THE FRONT DOOR TO NKFM

  • CIS is often the first point of contact for people seeking help.
  • The team advocates for individuals, supports families and caregivers, and connects communities to evidence-based programs with empathy and compassion.

MILESTONES THAT MATTER

  • Referrals:
    • Transitioned to ReferralMD to streamline communication and track referrals
    • Built partnerships with 10+ healthcare organizations
  • Resource Navigation:
    • Provided Finding Your Way (FYW) trainings statewide
    • Created an FYW book for Muskegon County

CELEBRATING ACCOMPLISHMENTS

  • Processed over 3,000 referrals in 2025 (and counting!)
  • Expanded resource navigation across all NKFM programs

70 Years of Impact: Disease Management Prevention – PATH Programs

Since launching its first workshop in 2007, PATH has expanded to include specialized programs for chronic disease, diabetes, chronic pain, kidney health, and high blood pressure — delivered in multiple languages and accessible formats. This work has helped thousands of Michigan residents build confidence, learn practical strategies, and take charge of their health one step at a time.

WHERE IT ALL BEGAN

  • NKFM launched its first PATH workshop in 2007 with Chronic Disease PATH, followed by:
    • Diabetes PATH (2009)
    • Chronic Pain PATH (2014)
    • Kidney PATH (2014)
    • High Blood Pressure Control (2021)
    • Tomando Control de su Salud (2023)

EMPOWERING SELF-MANAGEMENT

  • PATH helps participants learn to manage their health, build confidence, and connect with others.
  • It’s more than a class. It’s a movement of people learning to live well, one small change at a time.

GROWING, ADAPTING, & EXPANDING

  • The PATH suite has grown to include specialized programs for different conditions and communities. It is delivered virtually and in person.
  • Programs are offered in English, Spanish, Mandarin, and Arabic.

MILESTONES THAT MATTER

  • Added two extra sessions to create Kidney PATH
  • Piloted the nation’s first Spanish-language HBP Control program
  • Proved that virtual delivery of PATH was possible — now used nationwide
  • Secured 18+ years of continuous funding to sustain and grow programs

NKFM’s physical activity programs empower older adults across Michigan to stay strong, confident, and connected. From EnhanceFitness and A Matter of Balance to innovative new offerings like Bingocize, NKFM continues expanding accessible movement-based programs that support safety, independence, and whole-person wellness.

EnhanceFitness: STRONGER TOGETHER

  • Launched in 2008, EnhanceFitness offers low-impact aerobics that help older adults improve strength, balance, and energy at any fitness level.
  • More than exercise, it’s a community where participants motivate one another to keep moving and stay active.

BINGOCIZE: WHERE FITNESS MEETS FUN

  • Introduced in 2025, Bingocize combines light exercise, fall prevention education, and social connection — all built around a friendly game of bingo!
  • It keeps older adults moving, learning, and laughing together.

A MATTER OF BALANCE: CONFIDENCE IN EVERY STEP 

  • Started in 2014, this program helps older adults reduce their fear of falling and build practical skills for safety, independence, and peace of mind.
  • From home safety tips to balance exercises, A Matter of Balance helps participants stay steady and self-assured.

EXPANDING ACCESS, REACHING MORE PEOPLE 

  • Our physical activity programs continue to evolve. We offer virtual and in-person workshops, and now even Mandarin-language sessions for A Matter of Balance!
  • Every step forward means more communities gaining access to safe, inclusive wellness programs.

70 Years of Impact: Community Outreach

NKFM’s Community Outreach Team plays a critical role in helping Michiganders stay insured, informed, and connected to preventive health resources. Through multilingual education, strong community partnerships, and hands-on support, the team empowers individuals to navigate Medicaid changes, understand kidney disease risk, and make small everyday choices that lead to better health.

WHERE IT ALL BEGAN 

  • The Community Outreach Team was established in 2024 following the start of NKFM’s Medicaid Outreach efforts.
  • Its mission: to help Michiganders stay insured, stay informed, and stay healthy.

SUPPORTING COMMUNITIES THROUGH CHANGE 

  • After the COVID-19 public health emergency ended, NKFM’s outreach focused on helping Michiganders renew Medicaid eligibility and understand their health benefits.
  • The team also used this opportunity to expand education around kidney disease prevention and early detection.

STRONGER TOGETHER 

  • The team partnered with organizations like Centro Multicultural La Familia, Wayne Mobile Health Unit, St. Mary’s Catholic Church, LAHC, and Henry Ford Health to reach communities statewide.
  • They also helped develop educational materials in Spanish, Arabic, Mandarin, and Bangla, ensuring language is never a barrier to better health.

MILESTONES THAT MATTER 

  • Created a CKD training for Community Health Workers with MICHWA & earned the 2025 Outstanding CHW Champion Award
  • Attended 130+ community events in FY25
  • Supported screenings for 449 men at the Men’s Health Event at Ford Field

MAKING A DIFFERENCE 

  • Every day, the team meets people newly diagnosed with kidney disease who need answers, comfort, and guidance.
  • Through education, empathy, and connection, they help individuals understand their condition and find resources for better health.

70 Years of Impact: Early Childhood Education (ECE)

From statewide Go NAPSACC training to Regie’s Rainbow Adventure® and Farm to ECE, NKFM’s ECE team supports early learning environments that help Michigan’s youngest children grow up healthy.

WHERE IT ALL BEGAN

  • Mid-1990s – NKFM recognized that lifelong health starts early and began prevention programs for teens.
  • Early 2000s – Efforts expanded to preschool and K–12 settings.
  • 2005 – Early childhood programs became part of NKFM’s strategic plan.
  • 2000sRegie’s Rainbow Adventure® was born in Flint.
  • Following Years – MDHHS invited NKFM to implement NAP SACC (now Go NAPSACC), supporting childcare providers in building healthy environments.
  • Today – The Early Childhood Department continues to grow with Regie’s Rainbow Adventure®, Go NAPSACC, and Farm to ECE, helping children across Michigan start life healthy and strong.

MEET REGIE!

  • Regie’s Rainbow Adventure started as a fun way to teach kids about fruits and veggies.
  • Key Milestones:
    • Published a book documenting the program’s development and impact
    • Launched online sales and licensing
    • Created a Regie program video to support training and promotion
    • Developed three standalone books: oral health, social emotional well-being, and financial well-being

HELPING ECE PROVIDERS SUPPORT HEALTHY HABITS

  • Go NAPSACC helps childcare providers create healthier learning environments around:
    • nutrition, infant feeding, farm to ECE, oral health, physical activity, outdoor play, screen time
  • Key Milestones:
    • NAP SACC → Go NAPSACC transition in 2019
    • Expanded programming to Northern Michigan and statewide

CONNECTING KIDS TO LOCAL FOOD

  • Our Farm to Early Care & Education program helps children learn where food comes from through gardening, local produce, and hands-on workshops.
  • Key Milestones:
    • Became the leading organization for the MI Farm to ECE Network
    • Developed and hosted the Plant, Grow, Share: A Farm to ECE Workshop Series in 2025

  • Our Family & Youth programs were established around 2000, starting with programs like KICK (Kids Interested in the Care of their Kidneys) and KIDS (Kids and Kidneys).

  • The team’s work has evolved from classroom lessons to community-wide change, partnering with schools, families, and neighborhoods to make healthy living easier and more accessible.
  • From gardens and parks to after-school programs, their work inspires families to move more, eat better, and live well.

MILESTONES THAT MATTER

  • Expanded reach to more classrooms across Southeast Michigan
  • Built strong relationships with schools, families, and communities
  • Supported environmental changes like school gardens, active hallways, and healthy morning announcements

WORKING HAND-IN-HAND

  • The Family & Youth Team partners with residents, schools, and community groups to help them lead their own wellness initiatives.
  • One shining example:
    • Wildwood Elementary’s student-led garden club, where kids grow produce, they’re proud of and stay involved year after year.

STORIES THAT INSPIRE

  • One longtime partner shared that their child first tried hummus in an NKFM classroom over 10 years ago. Now that same family works with NKFM to improve physical activity opportunities in their community.
