Across Michigan, the National Kidney Foundation of Michigan (NKFM) offers free and low-cost programs designed to meet people where they are. Whether someone is managing a chronic condition, working to prevent disease, or simply trying to feel healthier day to day, our programs focus on access, support, and real-life change.

From multilingual workshops to virtual and in-person options, NKFM programs are built to fit people’s lives, not the other way around.

Personalized Support Through Health Coaching

NKFM’s Health Coaching Program offers one-on-one support to help participants set and reach personal health goals related to blood pressure, nutrition, weight management, and overall wellness.

Health coaches work alongside participants to create realistic goals and provide encouragement and accountability along the way. With bilingual coaches and strong partnerships with community organizations, the program helps reach individuals who may otherwise face barriers to care.

By collaborating with partners like local health departments and primary care centers, NKFM connects participants to additional resources and programs that support long-term health.

Education That Builds Confidence: PATH & HBP Control

NKFM delivers evidence-based self-management programs like PATH (Personal Action Toward Health) and HBP Control to help individuals manage chronic conditions such as diabetes and high blood pressure.

These workshops are offered in multiple languages, including Spanish, Arabic, and Mandarin, and are delivered in formats that work for participants — in person, virtually, or by phone.

Participants frequently report feeling more confident and informed after completing the programs.

NKFM also works closely with community partners to ensure programs are culturally responsive and accessible to diverse populations, including individuals with disabilities and those managing both physical and behavioral health conditions.

Practical Nutrition Education Through Cooking Matters

Cooking Matters at the Store helps participants build skills around grocery shopping, reading food labels, and preparing healthy meals on a budget.

Workshops are adapted to meet the needs of different populations, including young adults and individuals with or at risk for

 chronic kidney disease.

Participants often leave feeling more confident navigating the grocery store and making informed food choices.

Movement That Builds Strength and Independence 

The Walk With Ease program encourages participants to stay active through a supportive, structured walking program designed to improve strength, balance, and confidence.

Participants often share how completing the program positively impacts their daily lives, from feeling more energized at work to noticing increased ease with everyday movement.

NKFM continues to support Walk With Ease because of its proven impact and growing interest from participants across Michigan.
