Do you have kidney disease and want help managing it?

Kidney Personal Action Toward Health (Kidney-PATH) is an 8-week workshop for participants who are in various stages of Chronic Kidney Disease. The first six weeks focus on overall healthy living, with two additional sessions that cover kidney specific content.

The first six weeks were developed by Stanford University under the program name: Chronic Disease Self-Management Program. The last two weeks were developed by the NKFM.

What it is:

  • 8 weekly sessions (2 to 2 ½ hours each)
  • Fun, interactive, small group workshop
  • Led by certified leaders
  • No cost to participate
  • You will get a free book and program materials

Learn how to:

  • Manage symptoms and decrease stress
  • Improve communication skills
  • Avoid complications
  • Eat healthy with kidney disease
  • Understand treatment options, including dialysis and transplant
  • Ask a panel of experts more about kidney disease
  • Keep personal action plans – and more
